It is one of the most common major surgeries performed in obstetrics and gynecology worldwide. Despite its life-saving benefits, concerns remain about the knowledge, acceptance, and perception of CS among women of childbearing age. This study aimed to assess these factors among women attending a primary health center in Neni, Anambra State, Nigeria. A descriptive cross-sectional study was conducted using a semi-structured questionnaire administered to 112 women of childbearing age selected by simple random sampling. Data analysis utilized SPSS version 25. The majority (33%) of respondents were aged 25-31 years (mean 30.62 years), 60.7% were married, 49.1% had tertiary education, and 98.2% were Christian. While 64.6% had previously delivered vaginally, 35.4% underwent CS, primarily due to emergencies (72.4%). Most respondents (63.39%) demonstrated very good knowledge of CS, 19.64% good knowledge, and 16.96% poor knowledge. There was an overall positive perception towards CS, with 60.7% indicating acceptance for medical indications. Key reasons for acceptance included doctor''''s advice (88.2%), baby''''s safety (79.4%), and mother''''s safety (67.6%). Reasons for rejection (39.3%) were fear of surgery (77.3%) and high cost (54.5%). Influential factors on acceptance were family, friends, and religion. No significant association existed between knowledge/education and acceptance. this study found good knowledge, acceptance, and positive perception of CS among respondents, potentially attributable to their higher education levels. Identified barriers included culture, family approval, finances, and religion.
