It also informed the health officers on the causes of missing in the hospital with its possible effects on the healthcare outcome and ways management of patient health records needed for decision making. Among the objectives is to highlight the effects of missing records on patient care. Furthermore, to determine the effects of missing records on patients waiting time, and to suggest ways/ measures of checking and preventing missing records. Many medical and allied literatures were reviewed to develop more facts for the project work. This research use descriptive survey to sample 1313 staff of University of Ilorin Teaching Hospital, Kwara State. Questionnaire is the instrument used to gather and analyze the data, 153 questionnaires were distributed out of which 130 were also retrieved. In conclusion, recommendations were made that adequate infrastructural facilities and equipment should be provided so as to ensure adequate safety of patients’ records, staff should be trained and retrained so as to enable easy adoption of better ways of retaining records without loss. And improvement of the monitoring of patients' records should be put in place.
