Abstract
Transitioning from paper-based health records keeping to electronic health records (EHR) is a change process people's resistance to change is natural. Paper-based health records and electronic patient records generally are used in parallel to support different tasks in the hospital, therefore making the work tedious for the staff. (Muhammod–RabiuSalihu 2016) Objective: Objectives of the study is to examine the challenges face by health records professionals toward transiting from paper-based to electronic health records keeping. Method: This utilizes descriptive research design. Primary data was obtained through the use of questionnaires. Result:    The researcher deduced that the problem encounter with using paper based health records keeping and electronic/paperless health records keeping leads to breach of patient information and will not enhanced confidentiality with the percentage of 82.76% of respondents. This assertion correspond with (Peino, Linda, Hyde Cynthia 2006) which states that paper based health records keeping and electronic health record keeping leads to breach of patient information. Conclusion: Based on findings, the researcher concluded that The researcher has carefully analyzed the outcomes of this research work“assessing the challenges faced by health records professionals toward transitioning from paper based to electronic health records keeping” (A case study of Lagos University Teaching Hospital, Lagos State) as it relates to the quality of health services renders to patients. The following conclusions are hereby made: That the challenge encountered is that using both paper based and paperless health records lead to the breach of patient information. That the health information management department faces challenges such as unstable power supply, inadequate equipment e.g. computer sets, inadequate training towards transiting from paper based records to electronic health record keeping in Lagos University Teaching Hospital, Lagos State. ...
