It informed the health officers on the understanding of proper documentation in the hospital. Among the objectives is to investigate the effectiveness of proper documentation towards achieving quality health care and to evaluate if ignorance of staff and patients contribute to improper documentation resulting in lack or low quality health Care delivery in the hospital. Furthermore, to assess the usage of electronic health records software for documentation as it affects quality health care in the hospital. This research uses descriptive survey to sample staff of the University of Ilorin Teaching Hospital. Questionnaire is the instrument used to gather and analyze the data, 120 questionnaires were distributed out of which 120 were also retrieved.
