Clinical coding translates diagnoses into standardized codes to enable analysis. However, complex systems, lack of trained coders, and variability in practices challenge coding efficiency critical for HIMS quality. A questionnaire surveyed 52 health records staff on coding/indexing efficiency, methods, and barriers. Percentage analysis and chi-square hypothesis testing were applied. 71% used electronic medical/health records for coding but 36% reported low coding efficiency. 75% utilized ICD-10 yet only 39% felt it impacted health policymaking and 25% indicated no standardized methods. 77% agreed high coding quality enables analytics and research. Key challenges were complex systems (31%), coder shortage (29%), and lack of standardization (21%). Hypothesis testing using chi-square confirmed there are variable efficiency levels (X265.93.84). Despite adopting electronic records, clinical coding and indexing efficiency levels are inconsistent in enabling optimized health information management. Standardizing training, guidelines, and infrastructure is critical to improve effectiveness. The recommendations include Training programs for qualified coders, developing standardized guidelines, Complexity of systems requires evaluation, Enhance technology infrastructure, and policies supporting coding practices.
