A descriptive survey design was adopted for this study. The population of the study involved 805 targeted health workers. A sample size of 129 health workers, representing 16% of the population, was selected from the study area. Probability sampling method was employed with the use of stratified random sampling technique. Frequency distribution and simple percentage were employed for data presentation and analysis. Findings of the study revealed a generally positive perception of the performance appraisal system among employees, with a majority, 85.1% of the respondents, agreed that the criteria are clear, the process is fair, and the appraisals accurately differentiate performance levels and consider both quantitative and qualitative aspects. It was found that inadequate training for managers, unclear performance criteria, lack of regular feedback, inconsistent implementation, and insufficient time allocation negatively impact the accuracy and effectiveness of performance evaluations. This was agreed upon by 81.9% of the respondents. The findings revealed a strong consensus with the majority of respondents, 90.6% agreed on the positive impact of performance appraisals on employee motivation, personal and professional development, clear feedback, identification and rewarding of high performers, and fostering open communication between employees and managers.
