It also informed the health officers to identify problems associated with care outcomes when performing their legitimate duties. Among the objectives is to investigate problems faced by the patients with respect to seeking the services of Health Information Management staff. Furthermore, to identify possible solutions that could build practical, human and cordial relationships between patients and Health Information Management staff in the Hospital. Many medical and allied books were reviewed to develop more facts for the project work. This research uses descriptive survey to sample 70 staff and patients of the University of Ilorin Teaching Hospital, Ilorin, Kwara state. Questionnaire is the instrument used to analyze the data, 70 questionnaires were distributed out of which 62 were retrieved. In conclusion, recommendations were made that the staff of Health Information Management should have basic training this will eventually reduce the incidence of the consequence of care outcome.
