It informed the health officers on the influence of the tracer system as it relates to tracing patients’ folders in the hospital. Among the objectives is to determine the impact of the tracer system in controlling patient records and to find assess the effectiveness of the tracer system in health facilities. Furthermore, to examine the problem affecting and militating against the effective use of the tracer system. This research use descriptive survey to sample staff of State Specialist Hospital, Ikare Akoko Ondo state. Questionnaire is the instrument used to gather and analyze the data, 62 questionnaires were distributed out of which 52 were retrieved. In conclusion, recommendations were made that there should be a sound and well-organized health records department and dedicated health record staff so as to get patient records ready at due time, training should be conducted for untrained staff and/or trained personnel should be appointed. And adequate/enough staff should be employed in the health record department for the effectiveness of the tracer system and patient care.
