Abstract
The main objective of the study was to investigate the factors militating against effective documentation and implementation of Electronic Health Records (EHR) management in Nnamdi Azikiwe University Teaching Hospital. Specific objectives were to identify these factors, assess if there was proper EHR documentation, and investigate the impacts of effective EHR documentation and implementation. The research employed a quantitative approach. Data was collected through questionnaires administered to Health Information Management Officers at Nnamdi Azikiwe University Teaching Hospital and Odumegwu Ojukwu University Teaching Hospital. The scope was limited to these two hospitals. Key findings included the identification of factors hindering effective EHR documentation like inaccurate patient information, low internet speed, power supply issues, and lack of IT personnel and backup systems. It was also found that there were gaps in proper documentation processes for EHRs. Furthermore, the study highlighted the positive impacts of addressing these barriers such as improved patient-provider interactions and enhanced privacy and security of patient data. In conclusion, there are significant militating factors against proper EHR documentation and implementation. Overcoming obstacles like technological constraints and human resource limitations can lead to more accurate and holistic patient records, better coordination of care, and reduced medical errors. Key recommendations are increased government investment in hospital communication infrastructure, recruitment of more IT personnel, implementation of more robust backup systems and power supply, and training of Health Information Management Officers on EHR best practices. Addressing documentation issues is vital for improved patient care and outcomes....
