The following objectives were set: to determine the positive effect of health record delivery. To examine the factors that can impede Health Record Linkage in the Hospital, to ascertain the extent Records Linkage has helped in the achievement of an effective health care delivery system, to ascertain the methods of Health Records Linkage in the Hospital, to know the types of Health record on achievement of effective Healthcare delivery system. The research questions were formulated, Descriptive survey design was adopted in the actual execution of the study the population was 5,000 staff of FUTHA, and a total respondent of 360 was used as sample size. The Instrument adopted for the study was a structured questionnaire which was also used to calculate the Date of needed for the research, the following findings were made, that of a truth the impact of Health Record Linkage has a remarkable effect on patient Health care services, the need for employment of qualified professionals in Health Information Management Department is of a great importance.
