Abstract
This project examines the factors militating against effective registration and documentation of patient records in University of Benin Teaching Hospital. The objective of this research is to examine those factors militating against effective registration and documentation of patient records. The data of the study were sourced from both primary sources (self-developed and structured questionnaire), secondary sources (textbook, internet, observation). The research design used is the descriptive research design. Simple random sampling techniques was used to obtain the representative participant of 100 respondents drawn from health information management. Research instrument used for the collection of relevant data was a well-structured questionnaire, 100 questionnaire were administered and retrieved. Findings from the study revealed that poor communication and good public relation between patient and health record officer, ignorance, negligence, lack of transparency and trust between the patient and health record officer, hospital policy which has the highest percentage of the respondents response from the above table, and as the main factors militating against effective registration and documentation of patient’s records. The researcher recommends that there should be periodic workshops, seminar.  The research recommend that the use of EHR system should be integrated and utilized into the system of documentation to reduce ambiguous of proper based records which caused illegible handwriting, orientation, should be made for the patient by the hospital management and health record professional should be given necessary support, an incentives in order to encourage them work more effectively. ...
