Abstract
Management of femoral shaft fractures in children aged 6 months to 6 years typically involves closed reduction followed by immediate spica casting, a treatment that places considerable demands on families. This study aimed to assess the family impact of spica casting from an ecological perspective, using validated family functioning questionnaires. Pediatric femoral fractures are common and often effectively treated with spica casting, yet despite its long-standing use, there is limited formal guidance and no consolidated literature on optimizing outcomes. The study therefore reviewed existing evidence on spica casting for pediatric diaphyseal femur fractures (Heffernan MJ et al., 2021). Although fractures often appear well reduced in the operating room after spica application, bone length may subsequently angulate or shorten. To address this, the authors describe a technique involving a carefully molded hip spica cast designed to reduce the risk of revision surgery and malunion. Outcomes were compared between patients aged 1–6 years treated by a single pediatric orthopedic surgeon using this technique and those managed by other surgeons without it.
