Abstract
Nurses play a major role in the management of diabetic foot ulcers, which includes health education and patient care. The study aimed at assessing the determinants and outcomes of nursing management of diabetic foot ulcer among nurses and patients in the University of Ilorin Teaching Hospital. The Donabedian model was adopted. Concurrent triangulation mixed-method approach was adopted. Purposive and simple random sampling was used to select the ward and administer the questionnaire. Taro Yamane’s formula was used to select 91 participants from a target population of 105. Quantitative data was collected using a self-structured questionnaire with a reliability coefficient of 0.80 and analyzed using SPSS version 25 for descriptive and inferential statistics presented in tables and figures. Purposive sampling technique was used to select 6 respondents for the qualitative study. Qualitative data were collected using an interview guide and analyzed using NVivo version 15. The findings report that the majority of the nurses 95.6% had significant knowledge, 51.6% of the nurses had good knowledge, while 44% had fair knowledge. Using mean score, the findings reported key determinants to be poor patient adherence to the medication regimen(4.35) and poor glycemic control(4.10). These determinants were reported to have an influence on the outcomes of management, which are prolonged hospitalization(3.24) and unhealed wound(2.95). Qualitative findings highlighted themes such as preexisting knowledge of diabetes (4 patients) and financial constraints (3 patients) as determinants of patient adherence to management. It is therefore recommended that hospital administrators develop and implement policies to improve diabetic foot care....
