Abstract
Confidentiality is a cornerstone of medical ethics and a legal obligation that underpins trust in the patient–healthcare provider relationship. Protecting sensitive health information is essential for accurate diagnosis, effective treatment, and safeguarding patient rights. However, in many primary health care centers, particularly in resource limited settings, challenges such as overcrowding, inadequate training, and weak enforcement of ethical codes compromise confidentiality practices. This study assessed the awareness, attitudes, and perceptions of health workers regarding the ethical and legal implications of confidentiality in primary health care centers within Abia North Senatorial District, Nigeria. A descriptive cross-sectional design was adopted, targeting health workers across selected primary health facilities. Data were collected using structured questionnaires to evaluate awareness of confidentiality principles, attitudes towards maintaining patient privacy, and perceptions of barriers and facilitators to upholding confidentiality. Findings from the reviewed literature and preliminary evidence suggest that while many health workers demonstrate reasonable awareness of confidentiality standards, gaps remain in their attitudes and practical application. Factors such as years of practice, exposure to medical ethics training, and organizational culture significantly influence adherence to confidentiality. Moreover, perceptions of confidentiality are shaped by systemic challenges, including high patient load, limited resources, and the growing complexity of electronic health records. The study highlights the need for continuous professional education, stronger institutional policies, and supportive environments that reinforce confidentiality practices. By addressing these gaps, healthcare providers can strengthen patient trust, improve the quality of care, and ensure compliance with both ethical and legal standards. Ultimately, the findings contribute to policy development and capacity building initiatives aimed at enhancing confidentiality practices in Nigeria’s primary health care system. ...
