Abstract
Umbilical cord sepsis remains a major contributor to neonatal morbidity and mortality, particularly in low and middle income countries where harmful traditional practices and poor hygienic standards persist. Despite global recommendations for clean cord care, many mothers continue to apply unsafe substances such as cow dung, ash, toothpaste, and herbs, thereby increasing the risk of omphalitis and subsequent neonatal sepsis. This study assessed the knowledge, practices, and factors influencing umbilical cord sepsis among mothers attending selected primary health care facilities in Alimosho Local Government Area, Lagos State. A descriptive cross-sectional design was employed, with a sample size of 200 postnatal mothers recruited from Igando and Isheri Primary Health Care Centers using a purposive sampling technique. Data were collected through structured questionnaires and analyzed using descriptive and inferential statistics. Results revealed that 62% of mothers demonstrated poor knowledge of recommended cord care, while only 38% had adequate knowledge. In terms of practice, 69.8% of respondents reported applying substances other than methylated spirit or chlorhexidine to the cord stump, with toothpaste and local herbs being the most common. Educational status was significantly associated with cord care practice (p  0.05), as mothers with higher education were more likely to adopt hygienic practices. Furthermore, maternal age, parity, and antenatal clinic attendance were identified as predictors of cord care behavior. The study concludes that poor knowledge and harmful cultural practices remain key drivers of umbilical cord sepsis in Alimosho. Strengthening maternal education, promoting antenatal counseling, and enforcing evidence-based cord care guidelines are recommended to reduce neonatal infections and mortality. These findings provide critical insights for policymakers and healthcare providers in designing targeted interventions to improve neonatal outcomes in Nigeria. ...
