The study population for this research work comprised health information management practitioners in the health information management department of the hospital. The study employed a quantitative research approach and adopted a descriptive research design to provide insights, answer questions and evaluate the causes of misfiling among health information management practitioners and its effects on patients in the health facility. Questionnaire was personally administered by the researcher to 102 respondents, who served as the sample size for a population of 137 practitioners in this study. Data collected were presented in tables and analyzed using frequency and the simple Arithmetic percentage. The study discovered that delivery of quality healthcare services is affected by poor management of health records, particularly the misfiling of patients' health records. The study identified a lack of organization in the Health Information Management Department, keeping of inactive files in active cabinets, transposing of patients' numbers, nonchalance of workers, involvement of non-health records professionals and a poor filing environment as causes/factors responsible for misfiling among health information management practitioners in the University of Benin Teaching Hospital, Benin City. The study further examined the effects of misfiling of health records on patients. Increased patient waiting time, loss of trust in the hospital by patients, hindrance in continuity of care and death were discovered as the resulting consequences inflicted on patients due to misfiling of their health records.
