Factors militating against effective documentation and registration of patient’s record - case study of General Hospital Owo, Ondo State

Abstract
This study examined the factors militating against effective registration and documentation of patient records at General Hospital Owo, Ondo State. A descriptive survey research design was adopted. The population of the study consists of six-two (62) staff. The sample consisted of 55 hospital staff with direct access to patient record documentation and registration, selected using a simple random sampling technique. Data were collected from 55 healthcare staff using structured questionnaires (Likert-Scale). The data were analyzed using IMB SPSS (charts and mean statistics). The findings revealed that communication barriers, including language differences and complex medical terminology, significantly affect documentation processes. Time constraints and inadequate training were also identified as major challenges. Despite these barriers, healthcare personnel demonstrated a positive attitude toward documentation and recognized its importance in patient care. The study further revealed that while documentation materials are available, there is a significant lack of electronic documentation systems and supporting infrastructure. The study concluded that effective documentation and registration are influenced by both human and technological factors. It recommended the implementation of electronic health record systems, regular staff training, improved communication strategies, and investment in healthcare infrastructure to enhance documentation practices and overall healthcare delivery. ...
